Okay, this is a Pedro Almodovar movie which doesn't reveal its TG-ish-ness until 2/3 of the way in. But it's not exactly secret among our little subculture... but still, cut for spoiler...

The Skin I Live In has a guy wrongly accused of a crime to do with Antonio Banderas's mad scientist's daughter, kidnapped and surgically turned into a double of his late wife. The surgery is grounded, taking years piece by piece and requiring stretching exercises, but the results are still fantastical, with the character being played by a man in flashback and a woman in the present.
